Aged Care Reform Support At Home Program - 2025-




The Support At Home program, scheduled to be introduced in 2025, has seven categories of service providers.

The categories of service providers:

1. Personal and Domestic Assistance Providers

  • These providers offer support with daily living activities, including:

    • Personal Care: Assistance with bathing, dressing, grooming, and mobility.

    • Domestic Assistance: Help with household tasks such as cleaning, laundry, meal preparation, and shopping.

2. Nursing and Clinical Care Providers

  • Clinica healthcare services will be funded by the government regardless of the level of package that the older person is receiving.

    • Nursing Care: Wound care, medication management, chronic disease management, and health monitoring.

    • Clinical Services: Physical and mental health assessments, continence care, and management of complex health needs.


3. Allied Health and Rehabilitation Providers

  • Support services aimed at maintaining or improving physical and mental wellbeing, including:

    • Physiotherapy and Occupational Therapy: Assistance with mobility, fall prevention, and home modification recommendations.

    • Speech Therapy and Nutrition: Support for communication issues, dietary advice, and meal planning.

    • Social Work and Psychology Services: Addressing emotional, psychological, and social wellbeing.

4. Assistive Technology and Home Modification Providers

  • Focus on enhancing home safety and accessibility through:

    • Assistive Technology: Provision and installation of devices like mobility aids, alarms, and home monitoring systems.

    • Home Modifications: Construction or adaptation of the home environment, including installing ramps, handrails, or adjusting bathrooms and kitchens.

5. Community Engagement and Social Support Providers

  • Facilitate community participation and social engagement by offering:

    • Social Support: Group activities, companionship services, and volunteer coordination.

    • Transport Services: Assistance with transportation to appointments, social activities, or community events.

6. Respite and Carer Support Providers

  • Offer support to primary caregivers by providing:

    • Respite Care: Short-term care to give family members a break, either at home or in a community setting.

    • Carer Support: Educational resources, support groups, and counseling services for carers.

7. Case Management and Care Coordination Providers

  • Provide guidance and planning for older adults and their families, including:

    • Care Coordination: Development of individualized care plans and coordination of services from multiple providers.

    • Case Management: Regular review and adjustment of care plans based on changing needs and health conditions.

Each of these service categories is designed to address different aspects of ageing in place, ensuring that older Australians receive comprehensive and tailored support to live safely and comfortably in their homes.
